For Physicians
On our website, this section allows you to conveniently request an examination for your patient. Through our digital radiology system, your patient's imaging results are promptly accessible.
PACS ACCESS
Our clinics are completely digitalized with a Picture Archiving and Communications System (PACS). Physicians who refer patients can view their images from remote locations such as their offices or hospitals.
To obtain access to the physician portal, kindly send an email to Lori, our system administrator, at l.myers@xrayassociates.org and she will assist with setting up the access.
REQUISITION FORMS
In order to access the forms below, Adobe Reader is required. You can download Adobe Reader for free from www.adobe.com. We kindly request that you bring the completed form with you to our office at the time of your visit.
